Yungas tree frog

The Yungas tree frog, fasciated frog or spotted tree frog (Boana balzani) is a frog that lives in Bolivia and Peru. Scientists have seen it between 1200 and 2210 meters above sea level in the Andes mountains.[3][1]

This frog lives in cloud forests.[3]
