Source Scan via p. 31 of The Yale Younger Poets Anthology (1998). Because that scan was printed in black and white for the book, I've adjusted the hue in Photoshop to match a lower-res (but full-color) scan from Pazzo Books (direct link to jpg), which is also the original version of the image in the upload history below.
